Rarely, life-threatening squamous cell carcinomas may complicate discoid lupus erythematosus (DLE). Securing the diagnosis of keratinocyte carcinoma (squamous cell carcinoma more likely than basal cell carcinoma) within DLE may be challenging. Subungual tumors make me nervous. I realize that most are benign (subungual exostosis, osteochondroma, onycholemmal cyst, glomus tumor, pyogenic granuloma, wart, fibroma, acral fibromyxoma), however subungual malignant lesions (squamous cell carcinoma, keratoacanthoma, melanoma) occur. A delayed diagnosis can result in a profoundly adverse outcome.

I have used topical steroids under occlusion for as long I have been in dermatology. What a fabulous maneuver this is for difficult to treat dermatoses such as lichen simplex chronicus, recalcitrant psoriasis, or hypertrophic lichen planus, etc.

Allopurinol-induced adverse reactions: More than genetics

There are certain drugs that confer a high risk of severe cutaneous adverse reactions (SCAR) ranging from morbilliform drug eruptions, DRESS, Stevens-Johnson, to Toxic Epidermal Necrolysis. You know the usual suspects – anticonvulsive agents, sulfa drugs, other anti-infective agents, and antidepressants. Allopurinol is a frequent culprit.

There are multiple cutaneous manifestations of hemochromatosis, of which hyperpigmentation in sun exposed sites is most frequent. A significant association is with porphyria cutanea tarda.
