Step therapy
The Academy opposes step therapy and “fail first” strategies because they impede treatment and have been shown to have a negative impact on patients. Step therapy blocks patient access to medications, interferes with the physician’s decision-making, and undermines the doctor-patient relationship.

State advocacy win: Step therapy reform enacted in Alaska
A step therapy and prior authorization reform bill enacted in Alaska will require insurers to establish a streamlined prior authorization process that minimizes administrative burdens on physicians. The legislation provided a clear and convenient process for requesting exceptions to step therapy protocols and conditions for automatic approval of step therapy

AADA takes on step therapy reform, sunscreen access
New Jersey Gov. Phil Murphy (D) signed step therapy reform legislation into law, effective Jan. 1, 2026. The new law places guardrails around the use of step therapy protocols by state-regulated health plans, including Medicaid.

Low dose isotretinoin for recalcitrant rosacea: I pledge to use it sporadically
While rosacea can usually be reasonably managed with a combination of topical agents (metronidazole, azelaic acid, ivermectin, sulfacetamide, brimonidine), oral antibiotics (the tetracycline family), vascular lasers, intense pulsed light and surgery (for phymatous disease), some cases are recalcitrant to standard therapy.

Confluent and Reticulated Papillomatosis of Gougerot and Carteaud is usually seen in adolescents and young adults. Antibiotics (most commonly minocycline or doxycycline) are first-line therapy. The etiology of CARP remains to be determined, and further studies will judge its precise relationship to acanthosis nigricans.
Eruptive nevi: Trying to connect the dots
Eruptive nevi are striking – they may appear after a variety of disorders, including Stevens-Johnson/toxic epidermal necrolysis, primary blistering diseases, trauma, immunosuppression (from disease or iatrogenically-induced), PUVA therapy, or idiopathically.
